6.2.7.1 Screening of Patients for Transcatheter Mitral Valve Replacement

Because TMVR is currently in the clinical trial stage, there are no hospital clinical admissions, so relatively few clinical operations are performed. TMVR is confined to patients with advanced mitral valve disease who cannot tolerate open surgery. Such patients include (a) those with severe stenosis and valvular calcification who have moderate to severe valvular insufficiency and are not suitable for balloon angioplasty; (b) those with high-risk scores for cardiac surgery who cannot undergo surgery other than TMVR; and (c) those who are anatomically suitable for TMVR (mainly refers to the inner diameter of the mitral annulus and a peripheral artery within a suitable range). In this regard, a research team used 3D printing to produce a left heart model during systole and diastole based on patient-specific cardiac CTA data, which may include complex structures such as the left atrium, the left ventricle, the mitral valve and the chordae under it, the papillary muscle, the LVOT, the aortic valve and sinus, the ascending aorta, and the left coronary circumflex branch of the coronary artery. Through further analysis of this model, the researchers can more accurately screen patients and develop surgical strategies (Fig. 6.23).
