Discussion:

The GMC advises that doctors should not self-prescribe under most practical circumstances. This is obviously vital information, so answer 55 is A. While the UKCAT doesn’t expect you to know the GMC’s guidance, it’s reasonable for you to realise that if there is such guidance, it’s of vital importance.

The reasons why the GMC advises that doctors should not normally self-prescribe are that doctors cannot always be confident of their own diagnosis (making item 54 important, and answered as B), and that encouraging it can lead to public distrust in the prescription system, as well as creating a temptation for doctors to self-prescribe addictive substances.

The time it would save Greg and the GP is not completely unimportant; it would potentially help both Greg and the GP work more efficiently. However, this is only of minor importance (answers 53 and 56 being C), because other factors, such as professional guidance, weigh much more strongly.

Finally, antibiotic resistance is a real and growing issue. It’s not a vitally important issue in Greg’s dilemma, but is part of the reason for getting a GP’s opinion on the diagnosis before starting a course of antibiotics. Greg is a trainee eye surgeon, so is not as well placed as a GP to make judgements about starting antibiotics for a presumed chest infection. Therefore, the answer to 57 is B.

58, A; 59, A; 60, B; 61, D
