During the second week of October 1895, Sigmund Freud discovered the answer to an eternal question: How does the brain work?

How do thoughts happen? How do figments of the mind—wishes, words, fantasies, urges, memories—come into existence? Freud knew how, courtesy of a blinding moment of revelation, and he committed that knowledge to 102 handwritten pages that he mailed to a friend, along with an account of his epiphany: “The barriers suddenly lifted, the veils dropped, and everything became transparent.”1

For at least a century now, Freud’s primary public identity has been as the founder of psychoanalysis. Which he was. But this emphasis on his later career does a disservice to his earlier career, one that dominated the first twenty-plus years of his professional life, one that wound up influencing the next forty-plus years, and, in retrospect, one that might put his work on the unconscious into a fresh perspective. He was a neurologist.
