Perhaps the most complicated part of herpes today, aside from the diagnostic challenges and people not being aware they are infected, is sorting out type 1 from type 2. We are increasingly seeing type 1 genital infections and less often type 2 oral infections, and the “historical” anatomical divide between the two has been almost completely eliminated. Overall, type 2 is much more likely to be genital, and generally most genital infections are type 2. Here are some of the most common questions I get about type 1 and 2 herpes. The permutations are endless! (Well, not really endless, per se.)

Q: What if someone with type 1 herpes on their mouth has oral sex with someone who does not have herpes?

A: The person who is receiving the oral sex may get type 1 herpes on their genitals. This happens all the time.

Q: What if someone gives oral sex to someone with type 2 herpes?

A: They may get type 2 herpes on their mouth.

Q: What if someone receives anal sex from someone with a history of type 2 herpes?

A: They may get type 2 perirectal herpes.

Q: What if someone uses a sex toy from someone with a history of type 1 genital herpes?

A: They may get type 1 genital herpes.
