The name of the lay counselor or person to be rated is then filled in, with peer ratings by another lay counselor provided in three major areas, each with five items and thus with a total of fifteen items to be rated on the 1–10 point scale.

The first area rated is the use of supervision, and it contains the following items: (1) open to feedback from others, (2) uses time effectively, (3) communicates needs clearly, (4) appears able to understand the nature of the problem, (5) puts feedback to good use. The second area is counseling skills, with the following items: (6) demonstrates empathic ability, (7) shows nonjudgmental acceptance, (8) able to be appropriately confrontive, (9) communicates precisely and concretely, (10) able to develop rapport with clients. The third and final area is personal qualities, with the following items: (11) able to not be defensive, (12) approach to program consistent with training, (13) evokes confidence from others, (14) open to learning, (15) applies faith in a comfortable and appropriate manner.
