As she worked her calculator through Armando’s chart, Hazel saw no solution in the story of Dexter Advani. Armando does not qualify for Ben Taub, though if he did, she would have none of Dr. Conley’s qualms about sending him there. Nor was Armando’s family offering to take him home to Mexico. Cases such as his bring to mind an old Chinese proverb: “If you save a life, you are responsible for it.” Hermann saved Armando, and now it would have to care for him for as long as he lived.

For now there was little to do besides keeping track of his charges and suggesting ways to save a few pennies here and there. For instance, she noted on the chart, Lubriderm lotion costs about half as much as Nivea lotion. Perhaps the nurses could use more of one and less of the other?

About an hour after she began her initial review, Hazel put her findings into an empty folder, which she marked with Armando’s name and patient ID number. She would add to the file weekly despite the odds against any form of reimbursement to Hermann. She approaches cases like this with sardonic optimism. “Who knows,” she says. “Maybe he’ll win the lottery.”
