If we are to build systems that put people at the center of the work, we have to build systems that understand peopleâs stories in their complexity; in other words, systems that see people as fully human, complex, possessing multiple stories, and not easily categorized.
It is easy to suggest that scaled systems and organizations simply cannot understand individuals in the ways just described, but Alexander Packard is building a health-care system that does just that and proclaims it as its tagline: ârestoring humanity to health care.â At the heart of the approach is its use of health coaches, whose work is to bridge the distance between the busy physician and the patient. Similar to our use of academic coaches at SNHU or Groups Recover Togetherâs use of counselors in the lead care role, One Medicalâs health coaches get to know their patients in the fullness of their individual lives. As he explains:
The health coachâs job is to really understand the life circumstances, the well-being of the patient. Who lives at home with them? What are their hopes and dreams? Where are their barriers and obstacles? Whatâs going on in their lives? The abusive relationship, the addiction, the stress, the fact that they just got fired or their spouse is cheating or whateverâs going on in their life. That actually plays a huge role in our well-being. The classic doctor is, âHere, take this medication, change your diet, exercise more, and Iâll see you in three months.â In our model, if that happens, the health coach is there before the visit to get to know the patient. The first fifteen minutes are just the patient and the health coach talking. Then the doctor comes in and joins what we consider the patientâs team, and maybe brings in the behavioral health specialist or the nurse. The health coach has the last fifteen minutes of the visit, just one-on-one with the patient. A lot of this doesnât happen in traditional medicine. But whatâs interesting though is, if the doctor says, âHere, take this medication, exercise more, eat less and weâll see you in three months,â the health coachâs job is to say, âDo you have access to healthy foods? Do you have access to food? Can you stop smoking? What would prevent you from walking more? Do you live in a safe neighborhood?â
