Fetish

There is a small but significant population of people who require access to fetish objects in order to achieve sexual gratification. The objects may range from shoes and bras to balloons or body excreta; and we tend to evaluate the “severity” of a fetish based on the subjective social acceptability of the fetish object, rather than by evaluating the intensity of desire and impact of denial within the fetishist. Failing to engage with this issue in a person-centered way is a problematic approach. Maintaining a sense of genuine curiosity when exploring this topic with the client can help us gain a greater understanding of the fetish root and insight into whether this erotic attachment is seen as problematic by the client. Historically, the treatment for fetishism was aversion therapy, ranging in severity from inducing nausea to the application of electric shock (Lowenstein, 2002). These extreme treatments were not always predicated on client distress, but were often imposed in an attempt to force conformity with socially acceptable sexual practice. Today, the BDSM community can provide a safe and inclusive space for the fetishist to interact with their erotic object without stigma, which can reduce or even eliminate distress and isolation for our client, even if they do not otherwise identify as being BDSM.
