Ms. F, a most intelligent and articulate woman, aged 73, entered psychotherapy to deal with a sense of loneliness since the loss of her husband, dead five years, after a 15-year struggle with cancer. Mr. and Ms. F had been very happily married for 50 years, and had one son, a remarkable and successful performing artist. Ms. F moved into the city to live near her son, daughter-in-law and grandchildren. Given her age, many lifelong friends and relatives were deceased, and Ms. F suffered some age-appropriate social isolation. Ms. F responded well to individual psychotherapy and referral to, and completion of, group therapy in a short-term bereavement group, where she made a number of enduring friendships.
When hit with some health problems, especially a diagnosis of late-onset diabetes, she’s suffered adjustment disorder in the first two months after the diagnosis. Her symptoms are persisting longer than the above-noted six-month interval, since this disease has “enduring consequences.” She’s found it difficult to deal with new dietary programs, to negotiate and deal with her physicians, and to be appropriately assertive with the doctor, especially the new diabetes specialist. She’s needed her social worker’s atypical active intervention with her physicians as she struggles with new stressors, in taking care of herself, monitoring intake, weight loss, and the like. She can appear excessively exhausted, helpless, and depressed, and thereby disinclined to be active socially, and/or to finalize plans to visit welcoming relatives. Thus she suffers Bereavement V62.82, chronic type (longer than six months duration), and Adjustment Disorder with Mixed Anxiety and Depressed Mood, 309.28.
