We as readers knew she was afraid of being paralyzed
There was nothing she could do to save herself; there was no hope because her death was inevitable
She was vulnerable
There was a gruesome gore factor added on top of all the other psychological factors
Points 1, 2 and 3 are all emotional states, nothing to do with gore, and all to do with emotion. It’s the gore that gives the scare factor its context.
Psychological fear is about the emotional state your characters (and therefore your readers) are in.
