A knife-edged gingival margin.

Intact gingival margin (no ulceration).

A flat and triangular interdental papilla (normally filling inderdental space).

A probing depth <3mm, when 20–25g weight is applied to the probe.

Complete absence of any bleeding to probe.

Complete absence of any suppuration (pus).

A gingival margin situated just above the CEJ.

Complete absence of furcation exposure.

No recession.

It is important to remember, however, that clinical gingival health may not be classical (Fig 1-9). For example, teeth spaced apart may have a flattened interdental papilla. In addition, attachment loss may occur owing to toothbrush trauma and recession may be consistent with health, albeit not classical health. Following periodontal therapy, there may be reduced support, exposed furcations, residual drifting and mobility, but the disease may no longer be active and hence the tissues are clinically healthy at that time (Fig 5-3). Indeed, age-related changes such as recession and changes in soft-tissue consistency result in healthy periodontal tissues in the elderly failing to satisfy the conditions of classical or “pristine” health.
