8.2 The Validity of Dermoscopy in the Treatment of Telangiectasias

The dermoscopy exam performed immediately after a laser or IPL treatment states the reduction in number of the vessels in case of partial resolution, thus meaning that another session is suggested (Fig. 8.1c, d), and a disappearance of all telangiectatic structures in case of complete healing (Fig. 8.1e, f).

In case of telangiectasias sited on the lower limb, dermoscopy is essentially the choice of the gauge and the number of vessels which are usually not clearly visible to the naked eye.

Dermoscopy performed immediately after treatment shows the contraction of the vessels with total or partial disappearance of the treated vessels. Few days after, dermoscopy highlights some vessels’ damage in the form of small scabs, thus resulting from the necrosis of the vessels’ walls. The presence of small scabs along the length of the vessel is predictive of a good clinical result, which will be clearly visible 30–45 days after treatment. Meanwhile, it is important not to perform another session before this interval period in the aim to achieve a complete healing (Sevila et al. 2004; Piccolo et al. 2016).

On dermoscopy, telangiectasia highlights enlarged and broken superficial blood vessels and quantifies the number and gauge of the vessels to be treated as well as the concomitant presence of any photodamage (Fig. 8.2a, b).
