Alternatives for Postmenopausal Women

Because of the dangers of HRT, some postmenopausal women at risk for osteoporosis elect to take selective estrogen receptor modulators (SERMs) instead. In the late 1980s a group of breast cancer researchers noticed that postmenopausal women taking a SERM called tamoxifen for breast cancer were not losing bone mass. SERMs work by attaching to estrogen receptors in various body tissues and exerting a weak estrogen-like effect. They do not protect bone mass as well as estrogen itself does, but they also do not increase the risk of breast cancer. In 1998 the Food and Drug Administration (FDA) approved the first SERM for the prevention and treatment of osteoporosis. Researchers have found that this drug, called raloxifene (brand name Evista) reduces the incidence of spinal fractures in women with osteoporosis by 30 to 50 percent. Raloxifene is taken daily in pill form, and side effects may include hot flashes, leg cramps, blood clots, and flulike symptoms.
