This scale represents the maximum range of motion for the internal and external rotation of the femur in relation to the ilium. In this example the structure is held in a relative neutral position on the movement scale and has the ability to exert maximum internal and external rotation.

What happens subjectively when a structure is not in neutral, as in figure 14.5, is that the person describes the feeling as stiffness because they cannot exert the full range of the expected movement. In this example the “neutral” position of the hip is relatively more internally rotated. The experienced stiffness firstly depends on the fact that the person cannot exert more internal rotation between the femur and ilium as it is already, more or less, fully internally rotated. A person often expresses this as, “It feels like bone against bone.” Secondly, they also lack full external rotation, as there is no more length in the tissue to reach that far over the movement scale to the end of the external rotation. A person often expresses this as, “Look! I can’t go any further, I’m too stiff.”
