Palliative care in district nursing

During palliative care, district nurses spend time with the patient, listening to their fears and concerns and agreeing a plan of action where possible (Figure 36.1). The plan of action will not be to achieve a cure. The plan of action will be to address key concerns that the patient and those important to them have. This may be something physical – for example, pain control. It could be social – for example, Jan may be too breathless to be able to get up and down her stairs at home. It may be much more psychological and spiritual – for example, Jan may be asking why she got cancer when she has lived a healthy and ‘good’ life. Finally, it may be very practical forward‐planning – for example, thinking about where Jan would like to be cared for when she is approaching the end of life.

For some, a patient’s plan of action may be very limited, and may be as simple as enabling a patient to see their pet again, giving them peace of mind that their pet is being well cared for (Figure 36.2). For others, it will be much more extensive.
