This type of dialogue clearly states what you are asking for and your expectations. If the prospective mentor is not willing to provide what you need, this direct request gives them an easy opportunity to decline. If you have had a successful advisory relationship with someone and you approach that person about becoming a true mentor, do not be disappointed if they turn you down. Good mentors will only take on a few mentees. While they may have several advisory relationships, they will only have a finite amount of time to take a genuine interest and in-depth approach to someone else’s career given their own career, family, and personal demands. If they decline a mentoring relationship, continue to stay close to them and seek out another mentor internally or outside of the organization.

Your personal mentor is just that, personal. You do not have to advertise to others who your mentors are, nor do you have to let your mentors know if you have another mentor. Who you have chosen to help guide your career is your personal business, unless you choose to expose it. People in the organization will know who your mentor is if you have been assigned one, but again, if a mentor has been assigned, I would treat that relationship as an advisory one until you can access whether you have built a stronger bond with that person and they can help guide you in your career.
