Fig. 6.2A 68-year-old male patient with primary melanoma of the left shoulder resected more than 20 years ago demonstrated metastatic disease and was, thus, planned for therapy with the agent ipilimumab. Baseline 18F-FDG PET/CT before immunotherapy initiation (left) revealed a hypermetabolic, metastatic lesion in the left adrenal with a SUVmax of 24.7. After two cycles of treatment, early follow-up PET/CT (middle) showed a small decrease of the lesion’s metabolic activity (SUVmax of 21.3) and no further lesions. PET/CT at the end of the four-cycle treatment (right) demonstrated a complete metabolic remission of the adrenal metastatic lesion

Teaching point: This is a case of complete metabolic response under ipilimumab. According to interim PET/CT and based on the EORTC criteria, the patient would be classified as stable metabolic disease due to a less than 25% SUV decrease [58]. Nevertheless, the final outcome after completion of the four-cycle treatment proved to be complete remission, reflecting a clear response to the agent
