Last of all is the old chestnut (a subject, idea, or joke that has been discussed or repeated so often that it is not funny any more100), causality. As Peter Schönhöfer, Professor of Clinical Pharmacology, Zentralkrankenhaus, Bremen, Germany, said in a letter to the BMJ in 1997: “Drug companies and their experts use lack of causality in defending a product and to discredit reports of adverse effects. There is no causal explanation for the antidepressant effect of tricyclic agents, the antipsychotic effect of neuroleptics, or the sedative effect of barbiturates; there is only an association between the drug and the desired result. This is sufficient to grant therapeutic efficacy … (the) association does not simply disappear because experts declare that the events are not related causally. Such causality is misused as a bureaucratic safety measure.”101

Alcina Nicol, PhD, at the Instituto Oswaldo Cruz in Rio de Janeiro, stated in her independent overview of HPV vaccine that controversy still exists over whether the benefits of the available HPV vaccines outweigh the risks and this has suppressed uptake of the HPV vaccines in comparison to other vaccines. “Concerns about HPV vaccine safety have led some physicians, healthcare officials and parents to withhold the recommended vaccination from the target population.” Then she complained that “immunisation programs can be seriously compromised by safety … concerns.”102 This is true. Unfortunately, her message appears to be that the people should continue to be vaccinated irrespective of safety concerns about vaccines because immunisation programs are more important than the health of the recipients of the vaccines.
