Q. My boyfriend and I have tried to have sex a few times, but it hurts. What’s wrong? Is it possible that his penis is too big for my vagina? Or could it be something else?

A. It is possible that your boyfriend’s penis is too large for your vagina. With variances in both penis size and vaginal depth, some bodies just don’t fit together. While you can try different positions and ask him to be very gentle, you may not be able to get his penis all the way inside of you, though using a dildo on your own may help you get more used to the size and to the basic sensation of having something in there.

Other factors that may come into play are that you are tense for some reason (worried about privacy or pregnancy, for instance) and cannot produce enough natural lubrication; he may be entering you too quickly or too soon; or, because of your nervousness, your vaginal muscles tense up too much for penetration. Any of these things can cause sex to be painful.

Pain could also be an indication that something is not quite right physically. You could have a vaginal infection, which may or may not be a sexually transmitted infection (see pages 110–113). You could also have a condition that some girls have called vaginismus, which causes your vaginal muscles to involuntarily spasm, making sex very uncomfortable or impossible. Vaginal pain would then also be common during attempted pelvic exams. There are also certain nerve conditions that cause pain in the vulvar area that could make sex painful. These conditions are usually only temporary, but they do need treatment. If you suspect something is wrong “down there,” see a doctor as soon as you can.
