In order to avoid drug–drug interactions as far as possible while treating elderly patients, the following general recommendations can be applied to elderly schizo-obsessive patients as well (Mallet et al., 2007):

If possible, discontinue all the substances that interact strongly

Review all currently administered drugs for appropriate indication and target the lowest possible effective dose

Substitute the affected substance by another drug of similar efficacy but lower potential for interactions

Order monitoring of drug concentration

Be prepared to discontinue drugs rather than add new ones

Once an optimal medication profile is selected, observe the patient long enough

Exchange your experiences with other physicians and document the interaction management in order to promote a continuation of treatment.
