Until the late 20th century, anthropological studies often interpreted gender-diverse practices as personifications of same-sex desire.

Some studies focusing on Native American gender-diverse communities read these cultural traditions as representing homosexual lifestyles, particularly because two-spirit people who were biologically male but took on feminine characteristics often married masculine men, and vice versa. Some gay and lesbian members of Native American tribes choose to describe themselves as two-spirit people, aligning themselves with a cultural tradition of respect for alternative sexual or gendered lifestyles. Other anthropologists who instead represent them as transsexual cultures have questioned the positioning of the two-spirit people as gay forefathers.
