A Surgeon’s View of Injury and Healing

As part of my reflections on fracture, I spoke to the reconstructive plastic surgeon who put my jaw back together with metal bars and screws. Dr. Doug Courtemanche and I have developed a friendship over the years, as he has seen me now through three jaw surgeries: one that involved linking my split chin and broken condyles back together, another that (eighteen months later) involved breaking my jaw to improve its alignment, and a third that involved an emergency procedure to address an infection at the surgical site.

If you take a piece of wood and break it, you can’t put it back together the same way, because you get deformity and it splinters. Bones are the same. When you break a bone, you get a deformity. So, as a surgeon, if you just take the bone and put it back together, it’s not the same as it was before. So you have to account for that in the surgery. When you get a fracture, you never return to normal; you go to a new state. And as long as its form and function are appropriate, you’ll be fine. Medicine isn’t magical. People shouldn’t expect miracles and magic. They should expect practical improvements.
