Probiotics and Immunotherapy

There’s one more spoke on the immunotherapy wheel: adding a probiotic. Because research has suggested close ties between the gut microbiome and the immune system, it’s reasonable to wonder whether adding healthy gut flora to OIT could enhance the benefit. Researchers in North Carolina and Australia joined forces to see if combining peanut OIT with doses of bacteria called Lactobacillus rhamnosus could bring children with peanut allergy to sustained unresponsiveness. That “Holy Grail” of treatment results had yet to be attained when the study began, and the researchers hoped that adding a probiotic to OIT would tip the scales. According to their 2015 report, among the 28 participants randomized to the treatment arm (OIT plus probiotic), 23 had no allergic reaction to peanut up to five weeks after the study finished. Among the 28 randomized to the placebo arm, just 1 patient had that outcome. Not enough time had elapsed to know whether the tolerance remained, but the results were certainly intriguing. Four years later, the researchers reported that the benefit from the combination of OIT and probiotic had lasted. Probiotic research is tricky, but immunologists are continuing to explore the use of healthy gut flora as part of the quest to restore the immune system to its nonallergic state.
