• Adult/adolescent with or without HIV: PO/IM 5 mg/kg/day up to 300 mg/day or 15 mg/kg 2-3×/wk, max 900 mg 2-3×/wk

• Child and infant HIV positive: PO/IM 10-15 mg/kg/day max 300 mg/day
