This cyst is also complex in appearance with white strands that look web-like. Those strands are the ultrasound image of the blood clot trapped in the corpus luteum. This type of cyst is often referred to as “hemorrhagic,” meaning there is bleeding within the cyst.

Complex cyst – endometrioma

Endometriosis occurs when cells in the endometrial tissue (lining of your uterus) travel outside the uterus and implant in other areas such as the ovary or pelvis. These cells respond to normal ovulation hormonal changes, and they bleed just like you bleed during your menses or period. The implantation of this endometrial tissue outside the uterus can cause symptoms such as pelvic pain, menstrual cramps, painful intercourse, and infertility. Additionally, the bleeding can cause local inflammation, which leads to the development of adhesions in the pelvis.

An endometrioma is a cyst on the ovary that is a result from the accumulation of cells and blood. Endometriomas are commonly found in reproductive medicine and I see at least one endometrioma daily. Finding an endometrioma on ultrasound may help your provider explain your infertility. In general, complications from endometriomas are very rare. Endometriomas are described as a complex cyst, and they tend to have a classic dull gray or “ground glass” appearance. This type of cyst may have to be surgically removed depending on its size and symptoms.
