Integrating HIV with TB

Coordination, convergence, and integration are words most often used and least realized. In the roll-out of the HIV programme, evidence clearly showed that most HIV-infected people developed TB necessitating a close coordination with the programme. The TB division in the ministry was unwilling to cooperate and join forces with NACO. They were apprehensive that the stigma against TB, which was in decline, may get revived by its being associated with HIV/AIDS. Finally, the programme coordination was facilitated at the WHO’s insistence under the National Framework for Joint HIV/TB Collaborative Activities developed in 2007 that extended basic TB–HIV activities nationwide. An intensified TB/HIV package of services was developed to offer additional services in states with higher burden of HIV–TB. Coordination mechanisms such as the coordination committee and National Technical Working Groups (NTWGs) were established at the national and state levels. This programme was important as approximately 120,000 HIV-associated TB cases occurred in India annually,18 out of which an estimated 42,000 died each year, making India the second-highest burden country for people living with these co-morbidities. Case fatality rate among the notified HIV-positive TB patients in India is about 13 per cent, four times higher than that among HIV-negative TB patients.
