Case Analysis

The patient’s low-grade fever disappeared on the 3rd day after transfer and sputum disappeared roughly on the 5th day after transfer. The cough changed for the better on the 6th day after transfer and the patient had no obvious uncomfortable symptoms (like cough, sputum, and fever) when discharged. The changes showed that the symptoms were apparently improved by TCM.

The patient was diagnosed with AIDS, which was caused by HIV (corresponding to another kind of plague Qi in TCM). As Huangdi said, “five kinds of pestilence all can spread. No matter in children or in adults, the symptoms are the same.” However, HIV is obviously different from COVID-19 in transmission routes and intrusion modes. The routes of HIV infection are blood transmission, sexual transmission, and vertical transmission from mother to child. Plague Qi attacks blood collaterals (visible superficial vessels) easily. Besides, healthy Qi is gradually weakened and evil Qi is gradually vigorous with slow but steady progress of disease. In other words, excess of evil Qi and deficiency of healthy Qi were the basic states of the patient. Therefore, the patient was susceptible to infection. Then, he suffered from COVID-19, which entered through the respiratory tract and invaded the lungs. The patient complained that cough with white and frothy sputum could be caused by cold, and it was considered as deficiency of healthy Qi, especially lung-qi and wei-qi. Then, deficiency of wei-qi caused incursion of Cold Evil, which aggravated the vigorous state of evil Qi. In addition, it can be considered that the tongue color, which was slightly dark, was related with plague Qi of HIV and COVID-19. Plague Qi of COVID-19 attacked blood collaterals easily, because plague Qi of HIV invaded them first and resulted in deficiency of healthy Qi. Therefore, it was essential to unblock collaterals, promote blood circulation, and dissolve blood stasis as early as possible while taking the therapies of invigorating Qi, nourishing Yin, clearing away toxic material, and resolving dampness.

The core therapeutic principles in the case from the beginning to the end included invigorating Qi, nourishing Yin, clearing away toxic material, and resolving dampness. The principles conformed to the patient’s basic physical state and condition infected by COVID-19, which was in the condition of deficiency complicated with excess. In addition, it was necessary to adjust therapeutic emphasis flexibly (among clearing away toxic material, resolving dampness, invigorating Qi, and nourishing Yin) according to the manifestations in different phases.
