Urease production by some urethral microbes can result in the formation of urinary stones

Some UTIs are caused by bacteria that produce the enzyme urease and this can result in an additional complication—the production of urinary stones. These stones consist of ammonium urate, struvite (magnesium ammonium phosphate), and/or apatite (calcium phosphate). Although there are other causes of urinary stone formation, approximately 15% of stones are produced as a result of infection. Urease-producing organisms—mainly Proteus spp.—hydrolyze urea to ammonia and carbon dioxide, which makes the urine alkaline. Ammonium and bicarbonate ions are produced which then react with magnesium and/or calcium ions to produce complex insoluble carbonate and phosphate compounds. The resulting crystals often nucleate on the bacterial surface and then may grow rapidly to produce visible stones within 4 to 6 weeks (Figure 6.13).
