495 Complications Associated with Pregnancy

. Respirations less than 12 per minute, which can indicate impending respira-

tory arrest.

. Other signs of magnesium toxicity are visual disturbances, facial flushing,

GI upset, intestinal obstruction, and hypercalcemia. Because magnesium is

excreted by the kidneys, the client should be monitored for signs of renal

impairment.

If the nurse notes signs of toxicity, he should stop the infusion of magnesium sulfate and prepare to administer calcium gluconate. Safe administration requires the use of an IV pump or a controller. Common side effects of MgSO4 are drowsi- ness and hot flashes. Magnesium levels should be checked approximately every six hours and the results reported to the doctor.

NOTE

The treatment for magnesium sulfate toxicity is calcium gluconate. This medication should be kept at the bedside along with an airway and a tracheotomy Set.

NOTE

Fetal hypotonia and central nervous system depression can result from the use of magnesium sulfate.

Antihypertensive medications are also frequently used to control the client’s blood pres- sure during pregnancy. Beta blocker, angio-receptor blockers, and others categories might be ordered. Angiotensin converting enzyme inhibitors (ACE) are not generally recommended during pregnancy.

Ectopic Pregnancy

An ectopic pregnancy is defined as any pregnancy that occurs outside the body of the uterus. The most frequent site of implantation is the fallopian tube. Often the client does not know that she is pregnant, but she might have amenorrhea or experience some scanty vaginal bleeding. Tests for pregnancy are frequently positive. The diagnosis of an ectopic pregnancy can be made by ultrasound studies. Treatment includes laparoscopic surgery to evacuate the zygote. If the fallopian tube ruptures, bleeding and peritonitis can result. Scarring in the fallopian passage can result in an inability of the ovum to travel through the tube, resulting in sterility in the affected tube.
