Findings

Overall, women in the Canadian site were forthcoming with details of their pregnancies and birth experiences; there is clear evidence of a culture of sharing and analyzing these experiences. Transcripts tended to be very lengthy and detailed and provided for thick description and rich analysis. Canadian-born women expressed stronger preferences for midwifery care and tended to explain these preferences in terms of autonomy, woman-centeredness, and individual choice. Both Canadian-born and immigrant women expressed strong preferences for medical expertise and obstetrical care for reasons related to risk-reduction and safety. In exceptional cases where immigrant women did prefer midwifery care, this preference was explained with reference to family and tradition. The overwhelming majority of both Canadian-born and immigrant women stated clear preferences for hospital births. Among Canadian-born women (26), 19 preferred hospital births; 1 preferred the home as a birthplace; and 6 respondents were unclear about their preferences. Six stated preferences for midwives over other care providers; 18 preferred the care of physicians (either medical doctors or obstetricians); and 2 stated other preferences.2 Among immigrant women (95), 64 preferred hospital births; four preferred the home as a birthplace; 2 preferred a clinic; and 25 were unclear about their preferences. Nine stated preferences for midwives over other care providers; 82 preferred the care of physicians; and 4 stated other preferences.3
