Clinical considerations: Ascites

Pressure:

•Level 1 strokes or holds to lower and upper extremities and posterior trunk.

Site:

•Some patients will not tolerate touch to the abdomen. If they do, apply level 1 abdominal effleurage (in a clockwise direction), or abdominal holds may be comforting to some patients.

•Be mindful of skin integrity.

•Avoid areas where recent paracentesis was performed.

Positioning:

•Usually a Fowler’s (semi-recumbent) position, works best to ease shortness of breath. The angle of the head of the bed will depend on the patient. Let them be the guide to their own comfort.

•Traditional side-lying in a flat position is usually not tolerated due to breathing difficulties. However, for a short time, many patients find side-lying from a Fowler’s position allows them to breathe adequately while giving the therapist access to their back for massage. Also, it takes the pressure off the sacrum for a short while.

Other:

•A shorter session is often indicated.
