Lateral Wall MI’s are shown as ST-elevation on leads I, aVL, V5 – V6 with ST-segment depression in IIII and aVF. It is usually part of a larger anterolateral STEMI. The lateral wall of the LV is supplied by branches of the LAD and LCx. Complications include PVC’s (premature ventricular complexes, which is an early beat arising from the ventricles that looks wide and interrupts the P-wave) and LV dysfunction .

Posterior Wall MI’s are the mirror image of anterior MI’s. This is because a standard EKG does not have a direct view of the back of the heart. Tall, broad R-waves are seen in V1– 3 along with horizontally depressed ST-segments and upright T-waves. Posterior wall infarctions involve the LCx and RCA. Complications include PVC’s, AV block, and LV dysfunction. Posterior wall infarctions may be extensions of lateral and inferior MI’s.
