Depression, diabetes and heart disease represent three of the leading disease burdens in Australia with all three conditions being identified as National Health Priority Areas. Coronary heart disease (CHD) and diabetes frequently co-exist, and the impact of depression on both conditions has been shown to result in significantly worse mortality and morbidity outcomes as well as increases in health care costs. These chronic conditions are managed in the main in General Practice, and it is in this setting that the majority of patients seek help.
The international literature has shown that chronic illness care benefits from comprehensive and multifaceted health service delivery, and that General Practices are central to the success of coordinated chronic disease management (CDM). GPs often have limited time to manage the complex care needs of their patients and they could benefit from additional resources to help in CDM. The new Medicare Benefits Schedule (MBS) Chronic Disease Management Items introduced in July 2005 offer a potential solution but this requires changes in practice organization.
This research aims to: (1) develop and test a training programme for general practitioners and practice nurses in the screening, assessment and management of depression in patients with heart disease and/or diabetes; and (2) to test the feasibility of practice nurses screening, assessing, collecting data, counselling, referring, reviewing and monitoring patients with co-morbid depression and heart disease and/or diabetes. The design is a cluster-randomized intervention trial that will recruit 18 General Practices in order to generate 450 patients, with physiological, psychological and lifestyle measures taken at 3, 6 and 12 months. The primary clinical outcome will be a change in depression scores.
This project will lead to a tested model for assessment and treatment of co-morbid depression and heart disease and/or diabetes that is based both on sound research findings and the way that Australian General Practice is developing. The study will set up systems within General Practices to demonstrate how the care of co-morbid depression and heart disease and/or diabetes can be funded successfully using the new Medicare Benefits Schedule (MBS) Item Numbers.
