In exploring Ms. T’s background, she described her parents as highly focused on their own needs (“selfish” and “narcissistic”). The family operated on a pretense that everyone got along well, and any disagreements were unspoken or denied. Her father, a successful financier and her mother, a lawyer at a major firm, considered their careers to be primary and often worked late nights and weekends. To the extent they were involved with the family, they focused on the patient’s troubled brother, who suffered from impulse control and conduct disorder problems. On the few occasions when the children complained about their parents’ absence, they were lectured about how ungrateful they were about the “very nice life” their parents worked hard for them to obtain.
