Differential Diagnosis

• Complete ureteral duplication with upper pole ureterocele: The filling defect in the bladder has a typical appearance of a ureterocele, and the difference in the degrees of hydronephrosis between the upper and lower poles establishes the diagnosis of duplication with obstruction of drainage from the upper pole by the ureterocele. The inferior displacement of the lower pole collecting system by the obstructed upper pole is the “drooping lilly” sign.

• Bladder mass: Hematomas, fungus balls, and rhabdomyosarcomas can present as bladder filling defects, but they should not be anechoic centrally and are not associated with ureteral duplication.
