Following the interview, Lilja and the therapist completed the Intervention Planning form together. In collaboration they determined the type of adjustments needed to improve Lilja’s participation in school. An action plan stating the necessary steps and the team members involved was also formed (by whom, when it will be done, where, and how) (Figure 4-8). As many of the goals involved various team members, a meeting was scheduled with Lilja’s parents, her teacher, and her assistant to discuss the goals and to ensure that they were realistic and achievable within the timeframe required.
