BMD, by any of the techniques, gives three results for each site. These are an absolute value, expressed as grams of bone mineral per unit; a comparison to age- and sex-matched controls, or “normals“; and a comparison to young “peak” bone mass at age thirty. The results may be expressed as a percentage of normal, or as a standard deviation score, or distance from the mean value on the bell-shaped curve of normals. At first it does not make sense to compare a woman who may be fifty, sixty, or seventy years old with women aged thirty. The explanation is based on understanding that by age seventy to seventy-five, about 50 percent of all women already have thinned bones and are at risk for fractures. If we compare a seventy-five-year-old woman with other seventy-five-year-old women, she may be “average” for her own age group—but average at seventy-five is already high risk for fractures! We therefore compare women with an ideal group, thirty-year-old women, who do not suffer from these fractures. This comparison, when expressed as a standard deviation, is called a T-score.
