EMG responses may be detected during continuous free-running monitoring or during active electrical stimulation. Continuous free-running EMG is a passive way to monitor throughout the surgery. It is of greatest benefit during direct facial nerve manipulations, such as during acoustic tumor resection when surgical maneuvers may deform the nerve and create EMG potentials secondary to trauma. Passive monitoring is of lesser value when the surgeon has yet to identify the nerve.

Passive monitoring = continuous free-running electromyography during surgery

Active monitoring = passive monitoring plus active electrical stimulation
