SUMMARY

Contrary to conventional wisdom, there is an escalating incidence of substance abuse in older adults. The current cohort of older adults, the baby boom generation, is making a profound change in the culture of substance abuse in older adults. Unlike prior cohorts whose primary substance-abuse problem was alcohol abuse and dependency, the baby boom generation brings polysubstance experience. Projections indicate that the incidence of substance abuse will more than double by the year 2020.

This brings a special challenge to social workers who are the primary gatekeepers on the front line to recognize substance abuse and dependency in the older adults they serve and to implement treatment interventions. All social workers need additional training in substance-abuse treatment with older adults.

Another mission of these gatekeepers is to educate and coordinate with the physicians prescribing polypharmacy to their clients. In addition, an advocacy imperative is needed to change state laws to seek mandated centralized recording of controlled substances in a national database to eliminate the ability of older adults to seek prescriptions from multiple physicians, both in and out of state.
